Simple Fixation of Three Piece Intraocular Lens to the Posterior Surface of the Iris

Abstract:
Purpose: To present a simple and efficient surgical technique of three piece intraocular lens (IOL) fixation to the posterior iris surface in patients without capsular support. Methods: a prospective study of patients where capsular support was lost during phacoemulsification cataract surgery, and where the three piece IOL was fixated to the posterior surface of the iris. All patients underwent standard preoperative examination, and all had grade 2 and grade 3 cataracts. During phacoemulsification, a loss capsular support occurred and the IOL could not be implanted properly. After vitreous and remnants of the cortical material were cleaned from the anterior and posterior chamber, a simple technique is used to fixate the IOL to the posterior surface of the iris using 10-0 prolyne suture on a long needle. All surgeries were finished with the wound hydration and intracameral antibiotic instillation. Patients were followed one day after the surgery, and then again at one week and one month postoperatively. Uncorrected and best corrected visual acuity (UCVA, BCVA), possible myopic shift presence, intraocular pressure (IOP), position of the IOL and possible pupil ovalization were followed at each postoperative exam. For this study, result at one month postoperatively are presented. Results: Twenty five patients (27 eyes) in a period of one month were evaluated. At one month, mean value of UCVA was 0.64±0.11, and BCVA was 0.88±0.12. There was a mean value in myopic shift of -1.06±2.44. Intraocular pressure was normal in all cases, there was no IOL decentration, while four eyes had stretched pupils horizontally. Conclusion: IOL fixation to the posterior iris surface is a safe method in patients without proper capsular support. Future studies on a larger sample and longer follow up are needed to better understand this technique. Keywords: Cataract Complication, IOL Fixation, Technique.
